This order form is for chapter gift book orders only. If you would like the chapter to pick up their book at Convention, there is no shipping charge – just check the “Pick up at Convention” option below, and the chapter delegate will receive the book and a note that you have gifted the book to the chapter at the registration booth on site. If you would like the book to be shipped to the chapter, check the “Shipped” option below, and the book will be delivered to the address on file for the chapter. You do not need to look up the address in advance.
